To schedule an appointment or to obtain additional information about services, please fill out the form below or give me a call. If you will be using your health insurance, please indicate what insurance you have in the “comments” section.

Internet Policy
I will only use the internet for communication about appointments. I do not use the internet to communicate with clients or prospective clients for therapy because the laws regarding the therapist’s responsibility for confidentiality in internet communications with clients have not yet been addressed.

Cancellation Policy
As the scheduling of an appointment involves the reservation of time specifically for you, you must give at least 24 hours notice to cancel an appointment. If this does not occur, you may be charged a late-cancellation/no-show fee. Insurance companies do not reimburse for missed sessions.
